I was going to make an appointment for applying but wanted to ask what kind of information I will need to bring with me. I was just approved for Medicaid and I am 9 weeks pregnant.
Hello! I have a 6 month old daughter and while I was pregnant with her I received formula samples in the mail. She is breastfed and I have never needed them. Could I donate them to your office? Also do you work in affiliation with any milk banks? I have a large frozen stash of breast milk that I would like to see go to someone in need. we live in woodridge.
